Description :

The position involves providing home health care to patients in their residences.

Candidates must have experience in cardiopulmonary assessments and basic wound care.

Responsibilities include performing ABI compression dressing, wound vac dressing, and Pleurx / Asept drain care.

Home health duties will also involve foley catheter changes, staple / suture removal, and lab draws via venipuncture.

The role requires skills in IV management, including PICC line or midline care and dressing changes.

The nurse will be responsible for administering SQ and IM injections.

Candidates should have experience with general medical diagnoses and high-risk diagnoses such as heart failure, COPD, myocardial infarction, pneumonia, and stroke.

Post-operative care experience is required.

Daily skilled home visits will range from 6 to 8.

Scheduling flexibility is necessary, as hours may vary to meet patient and office needs.

All nurses must hold a valid driver's license and have a car for home visits.

COVID-19 vaccination or exemption is mandatory to work.

Flu vaccination or declination form is also required, with mask-wearing during work hours.

Recent patient care experience within the last three months is essential.

Strong IV, phlebotomy, and wound care skills are needed.

Candidate must also be proficient in patient assessment documentation and routine care.

The position requires adherence to all infection control protocols and healthcare regulations.

The ideal candidate is expected to work collaboratively with interdisciplinary teams, including social services and pharmacy support.

The organization promotes a smoke-free environment for all staff and patients.

Compliance documentation will be required before the start of the role.

This is a traveling position, with the potential for floating to nearby facilities if needed.
