Afterhours Care Manager Registered Nurse

The Afterhours Care Manager Registered Nurse plays a vital role in delivering comprehensive care by addressing patient and family needs during after hours and facilitating effective care management within a dedicated team. This position operates on an Alternate Schedule : Monday, Tuesday, Friday, Saturday & Sunday one week, and Wednesday & Thursday the following week.

• The role involves managing patient care through routine visits, admissions, death visits, on-call or after-hours visits, and any duties aligned with the patient's care plan.
• Provides after-hours coverage for designated teams and serves as an interdisciplinary team member to ensure seamless communication among physicians, patients, and the hospice team.
• Responsible for clinical administrative tasks that include workflow completion, auditing, and maintaining survey readiness.
