The Afterhours Care Manager Registered Nurse plays a vital role in delivering comprehensive care by addressing patient and family needs during after hours and facilitating effective care management within a dedicated team. This position operates on an Alternate Schedule : Monday, Tuesday, Friday, Saturday & Sunday one week, and Wednesday & Thursday the following week.
- The role involves managing patient care through routine visits, admissions, death visits, on-call or after-hours visits, and any duties aligned with the patient's care plan.
- Provides after-hours coverage for designated teams and serves as an interdisciplinary team member to ensure seamless communication among physicians, patients, and the hospice team.
- Responsible for clinical administrative tasks that include workflow completion, auditing, and maintaining survey readiness.
Qualifications :
Must hold a valid RN license in the state of employment.Graduate of an accredited nursing program.At least one year of experience as an RN in acute care, oncology, long-term care, hospice, or home health settings.Licenses and Certifications :
Current nursing license to practice in the specified state.CPR Certification (in-person).Valid driver's license, current automobile insurance, and reliable vehicle access.Skills and Knowledge :
Thorough understanding of hospice principles, regulatory standards, and comfort care practices.Proficiency in nursing assessments and interventions.Adept at meeting the diverse needs of hospice patients and their families.Strong time management and priority-shifting abilities in a rapidly changing environment.Exceptional collaboration and communication skills across various teams and stakeholder groups.Personal Attributes :
