Home Health Registered Nurse Case Manager

Currently we are assisting one of our top home care agency partners with a need for an experienced Home Health RN Case Manager .

Coverage territory needs include Chatham / Effingham / Bryan counties.

• Monday-Friday
• Full-time, Contract
• Occasional on-call if needed
• Competitive pay rates plus mileage reimbursement

The Home Health Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN's and Home Health Aides.

Requires minimum of 1 year of recent Home Health RN Case Management experience doing OASIS visits with a Medicare certified agency.
