Become the Director of Case Management You've Always Dreamed Of

Take responsibility for the daily operations and human resource management of the Case Management department.

Oversee the interdisciplinary plan of care and discharge planning processes, ensuring effective services focusing on census management, patient care outcomes, and key care indicators.

Act as an advocate for patients and families, ensuring that services meet their unique needs.

Provide support and guidance to Case Managers and staff, including training on caseload management and understanding regulations, policies, operational procedures, and objectives. Continuously review operations to maintain high standards of quality in line with organizational expectations.

Build strong relationships with insurance companies, self-insured employers, case management firms, and other healthcare networks.

Qualifications

Current CCM® or ACMTM certification is preferred.

Must be qualified to independently conduct assessments within the scope of practice of your discipline.

If licensure is required for your discipline within the state, you must hold an active license.

For Nursing, a bachelor's degree in nursing (BSN) with RN licensure is required.

For other eligible healthcare professionals, a minimum of a bachelor's degree is required; a graduate degree is preferred.

Three years of hospital-based Case Management experience, inclusive of Utilization Review and Discharge Planning expertise.

Willingness to work on weekdays and / or weekends, evenings, and / or night shifts as needed.

May be required to work on religious and / or legal holidays during scheduled days / shifts.
