Job Description
Aleria Home Care Agency is hiring a Part-Time Care Coordinator to help ensure our clients receive compassionate, reliable care. This position is ideal for someone who is highly organized, passionate about serving others, and looking to make a meaningful impact with flexible scheduling.
Key Responsibilities
- Conduct new client intakes and assist with developing non-medical care plans
- Coordinate caregiver schedules and ensure proper client-caregiver matches
- Provide support and direction to caregivers regarding care needs and shift updates
- Communicate with clients and family members regarding ongoing care and service changes
- Monitor documentation, client updates, and ensure compliance with agency procedures
- Manage client and caregiver concerns with empathy and professionalism
- Assist with after-hours scheduling needs and backup on-call (as needed)
Qualifications
High school diploma or GED required; Associate’s or Bachelor’s in healthcare, social work, or similar field preferred1+ years of experience in home care, case management, or caregivingKnowledge of scheduling, care documentation, and client service practicesStrong written and verbal communication skillsBasic computer skills and familiarity with Microsoft Office or scheduling systemsCPR certification (or willingness to obtain)Valid driver’s license and reliable transportationWhy Work With Aleria?
