Job Title : Supportive Housing Manager – Farmington Supportive Housing Complex
Location : Farmington, CT
Employment Type : Full-Time, (FLSA) Exempt
Schedule : 1st Shift, M-F, Flexible, including evening and weekend availability to support needs of program (40 hours / week)
Salary : $ 60,000 annually
Reports To : Supportive Housing Coordinator
Department : Residential Supportive Housing Services
About the Role :
Favarh is seeking a compassionate, independent, creative and organized Supportive Housing Manager (SHM) to lead our brand-new inclusive supportive housing complex in Farmington, CT . In this role, you'll work closely with our Supportive Housing Director and Supportive Housing Coordinator to oversee operations, supervise staff, coordinate individualized supports (transportation, employment), and ensure the success of this community-based housing initiative for adults with intellectual and developmental disabilities (I / DD) while adhering to DDS policies and Favarh standards. This is Favarh’s sixth inclusive housing development, offering affordable, integrated, person-centered living options in the heart of Farmington Valley and surrounding towns.

Key Responsibilities :
Program Operations & Participant Support :
Oversee daily operations of the Farmington Supportive Housing programCoordinate & Oversee tenant move-ins, orientation, and transition planningMaintain 24-hour on-call availability per Emergency On-Call ProceduresEnsure apartment conditions, safety standards, and personal care supplies are maintainedAssist all individuals in the art of living in their own apartment and being a good tenant.Create and manage individualized schedules for ADLs, transportation, employment, recreation, and medical appointmentsProvide life-skills coaching and individualized support; refer clinical concerns as neededEnsure timely completion of medical, psychiatric, and dental appointments according to DDS guidelines and individuals health and medical needsCoordinate and maintain participant records in compliance with DDS standardsAssist in habilitation and case coordination activities as neededAssist individuals with respect to financial oversight, including budgeting, bill paying and redeterminations as assigned.Staff Supervision & Development :
Directly supervise Supportive Housing Assistant Manager, Medical Liaison, and DSPsParticipate in hiring, training, scheduling, and performance management of staffProvide ongoing training and “on-the-floor” coaching in accordance with Favarh and DDS Quality Assurance guidelinesEnsure staff compliance with Medication Administration, First Aid / CPR, PMT, and all mandated training coursesQuality Assurance & Compliance :
Utilize & monitor documentation in Therap to ensure complete and accurate billing (EVV), service delivery, and daily logsMonitor participant finances and ensure spending aligns with approved budgetsParticipate in PRC / HRC process, reviews, audits, and other regulatory processes as assignedMaintain program documentation, safety protocols, and purchasing in line with agency policy and DDS regulationsMonitor use of agency vehicles and ensure driver compliance with Favarh policyEnsure EVV documentation aligns with Therap recordsManager is responsible for ensuring that documentation within Therap and in personal (hard copy) files is complete, accurate, and maintained to meet the requirements of the (DDS) licensing and Quality Service Review (QSR) .Communication & Community Engagement :
Maintain regular communication with families / guardians, team members, internal departments and external service providersCoordinate participant and family orientation to the programAct as liaison between families, healthcare providers, neighbors, local groups, and the broader community to promote inclusion and awarenessSupport participant engagement and integration into the local community through social and recreational planningPromote Favarh’s mission and values to neighbors, stakeholders, and the publicMinimum Qualifications :
Bachelor’s degree preferred or equivalent experience in human services, residential, or IDD-related fieldMinimum 3 years of supervisory experience in a residential or supportive housing settingMedication Administration Certification (required within 3 months of hire)Certifications in PMT, CPR / First Aid / AED and DDS Medication Administration Certification (or must obtain within 60 days)Familiarity with DDS regulations, EVV, Therap, Microsoft Office, Excel and electronic scheduling systemsStrong organizational, interpersonal, and communication skillsValid Connecticut Driver’s License with a clean driving record (REQUIRED)Must meet Favarh transport driver approval (Ride Share eligibility)Physical & Work Environment Requirements :
Ability to stand, walk, and move frequently during program oversightMust be able to operate agency vehiclesOccasional lifting / moving of materials may be required – must be able to lift up to 75 lbs.Work takes place in apartments, community settings, and office environmentsWhy Join Us?
