About the Role

The Company is in search of a Vice President of Care Navigation to lead the implementation of new partner integrations, with a strong focus on data-driven, proactive clinical intervention and a compassionate patient experience. The successful candidate will be responsible for the entire implementation lifecycle, from pre-signature planning to post-signature execution, and will work to enhance the Model of Care in collaboration with partners. This role requires the design and refinement of patient-centric clinical programs, the establishment of evidence-based workflows, and the implementation of scalable care navigation strategies. The VP will also be tasked with inspiring and managing high-performing clinical and non-clinical teams, ensuring compliance with regulations, and driving the adoption of AI and automation tools to improve clinical decision support and care coordination. Applicants for the Vice President of Care Navigation position at the company should have a minimum of 10 years' progressive leadership experience in clinical operations or related healthcare domains, with a proven track record in mid-to-senior management roles. A Master's degree in a relevant field is required, with an advanced clinical degree preferred. The ideal candidate will have a deep understanding of care coordination requirements across various healthcare programs, clinical proficiency in managing mental health and chronic conditions, and experience in designing and scaling patient-centric clinical programs.
