What You’ll Do

As an RN Case Manager , you’ll play a vital role in ensuring quality patient care and supporting families throughout the care process. You will :

Develop, implement, and evaluate individualized plans of care

Coordinate and oversee home health services for assigned patients

Review and submit clinical documentation and billing accurately and on time

Communicate with physicians, ADONs, DON, and support staff to ensure continuity of care

Educate patients and families on health management, medications, and home care needs

Attend MD appointments and advocate for necessary DME, medications, and services

Provide clinical guidance and mentorship to field staff

Maintain patient information in QuickBase and ensure compliance with agency standards

Participate in case management meetings, continuing education, and company initiatives

Qualifications

Active Compact RN License (Tennessee or multistate)

Minimum 3 years of clinical nursing experience (home health experience preferred)

Strong organizational, documentation, and communication skills

Prior leadership or management experience a plus

Proficient with computers and electronic documentation

Compassionate, reliable, and team-oriented demeanor

Ability to safely lift 40–50 lbs and perform hands-on patient care

Professional appearance (scrubs and name badge required)
