• We are currently hiring a Community Health Worker – C3 Care Management based in Worcester.
• As part of the care management team, the Community Health Worker (CHW) supports individuals with complex or chronic conditions who face barriers to care.
• The role is flexible and changes day-to-day to meet members’ needs.
• Outreach may be done by phone or in person in various settings, including the community, homes, facilities, or the health center.
