Minimum of one (1) year nursing experience, preferably in a Home Health / Hospice setting within the past five (5) years is preferred. Certification in Hospice and Palliative Nursing a plus but not required. Current CPR certification. Must have a valid driver’s license and auto liability insurance. Understanding and acceptance of hospice philosophy. Strong clinical, organizational and interpersonal skills. Strong written and oral communication skills.

Join our compassionate team as a full-time RN Case Manager! You'll be the heart of hospice care, coordinating care for patients and families during their most important time. What You'll Do :
- Manage 10-14 patients, ensuring they receive appropriate care.
- Complete assessments, create care plans, and document care.
- Collaborate with doctors, social workers, and other team members.
- Provide support and education to families.
- Enjoy flexible scheduling and work-life balance (avoid burnout!).
