Job Description :

Supplemental Health Care is currently hiring Social Workers with previous OASIS experience for home health contracts in Eugene, Oregon, and the surrounding areas.

Home Health Social Worker Contract Details :

1, - $1, per week
SHC Home Health Social Workers are paid by the hour, not by the visit
Social Worker will have a designated territory of coverage
Weekly expectation is 30 units of productivity
Home Health contracts with SHC require previous experience with OASIS / Medicare Certified Visits
Estimated weekly payment may include both taxable wages and tax-free reimbursements for meals, housing, and incidentals. Please speak with a recruiter for details.

Requirements :

OASIS / Medicare Certified Visits Experience

Electronic Charting experience

Minimum of 1 year of recent home health Social Worker experience (private duty does not qualify)

Most contracts require some on-call

Current Oregon Social Worker License

American Heart Association BLS

Valid U.S. Driver's License

Supplemental Health Care will consider for employment qualified applicants with criminal histories in a manner consistent with the requirements of applicable local, state, or federal law. For this type of employment, Kentucky state law requires a criminal record check as a condition of employment.
