Reliance Community Care Partners has a unique opportunity for a fully licensed Social Worker (LMSW) to use their gifts and experience while not getting in the way of a healthy balance of work and life! This position offers the opportunity to work from home locally while residing within western Michigan as well as assessments out in the community and the opportunity to work as an essential part of an interdisciplinary health care team. Territory is the following counties : Kent, Muskegon, and Ottawa.

What Will You Do?

In this community-based program, the MSW OBRA Evaluator is responsible for performing in-person assessments for individuals who may have been diagnosed with a mental health condition and / or developmental disability and may require placement within a long-term care facility. An assessment is completed to determine the level of care, as well as to make recommendations for both nursing and mental health services.

Education and Experience

  • MSW full licensure in the State of Michigan (LMSW)
  • At least 2 years of clinical experience, including mental health experience
  • Older adult care or experience working with disabled population preferred
  • Reliable transportation to make client assessments
