Registered Nurse (RN) Case Manager

This position pays $42-46 per hour DOE

Join our compassionate team as a full-time RN Case Manager! You'll be the heart of hospice care, coordinating care for patients and families during their most important time. What You'll Do :

• Manage 10-14 patients, ensuring they receive appropriate care.
• Complete assessments, create care plans, and document care.
• Collaborate with doctors, social workers, and other team members.
• Provide support and education to families.
• Enjoy flexible scheduling and work-life balance (avoid burnout!).
