Job Summary We are seeking a compassionate and dedicated Hospice Nurse to join our team. The ideal candidate will provide high-quality nursing care to patients with life-limiting illnesses, ensuring comfort and dignity in their final days. This role involves working closely with patients, families, and interdisciplinary teams to deliver holistic care tailored to individual needs. The Hospice Nurse will utilize their clinical expertise to manage symptoms, provide health coaching, and support families through the end-of-life process.
Responsibilities
Conduct comprehensive physical examinations and assessments of patients in hospice care.
Develop and implement individualized care plans in collaboration with the healthcare team.
Provide case management services, coordinating care across various settings including outpatient facilities and Level I Trauma Centers.
Administer medications and treatments, utilizing aseptic techniques to prevent infection.
Educate patients and families on disease processes, treatment options, and symptom management.
Offer support for individuals with developmental disabilities as part of a holistic approach to care.
Document patient progress using eClinicalWorks or similar electronic health record systems.
Collaborate with interdisciplinary teams to ensure comprehensive infectious disease care when necessary.
Participate in ongoing training and professional development opportunities related to hospice care.
Experience
Previous experience in hospice or palliative care is preferred but not required.
Familiarity with health coaching principles is a plus.
Experience working in a Level I Trauma Center or similar high-acuity environment is advantageous.
Demonstrated ability to manage complex cases involving infectious diseases or developmental disabilities.
