Case Manager
Location : Van Nuys

Pay Rate : $25-$27 / hour
About the Role :
LHH is seeking a compassionate and detail-oriented Case Manager to support our mission in serving veterans at risk of homelessness . This position plays a vital role in helping clients navigate housing, benefits, and essential services. The ideal candidate will have strong administrative skills, empathy, and the ability to manage multiple client cases with professionalism and care.
Key Responsibilities :
- Manage an active caseload of clients, primarily veterans at risk of homelessness
- Conduct intake interviews and guide new clients through the application process
- Maintain detailed and accurate case notes and documentation in the system
- Provide housing navigation support, including apartment searches and lease application assistance
- Assist clients with completing various documents such as bank account information and check requests
- Coordinate and process client requests for financial assistance and other resources
- Collaborate with internal departments and external partners to secure housing and services for clients
- Ensure clients are connected with appropriate resources to support long-term stability
Qualifications :
1–2 years of experience in case management, social services, or a related fieldExperience working with veterans or vulnerable populations preferredStrong administrative and documentation skillsProficiency with data entry, online research, and Microsoft OfficeExcellent interpersonal, organizational, and problem-solving skillsAbility to maintain confidentiality and manage sensitive informationBilingual (Spanish / English) is a plus
