GENERAL SUMMARY OF DUTIES : The Lead Care Manager is responsible for coordinating and overseeing the care of patients within a healthcare setting, ensuring that all care services are delivered effectively and in alignment with patient needs. This role involves managing a team of care managers and care coordinators, liaising with healthcare providers, and ensuring that care plans are created, implemented, and updated as needed. ESSENTIAL FUNCTIONS :
1. Care Coordination : Oversee and manage patient care plans, ensuring they are customized based on patient needs and preferences.
2. Team Leadership : Lead and supervise a team of care managers and care coordinators by providing support, guidance, and mentorship.
3. Collaboration with Providers : Work closely with physicians, nurses, and other healthcare professionals to ensure that care plans are aligned with medical recommendations.
4. Patient Advocacy : Act as an advocate for patients, addressing any concerns or issues they may have and ensuring that they receive quality care.
5. Assessment and Evaluation : Conduct patient assessments to determine care needs and make recommendations for necessary services or adjustments to care plans.
6. Education and Support : Educate patients and their families about care plans, treatment options, and available resources.
7. Documentation and Reporting : Maintain accurate records of patient care plans, updates, and progress. Ensure all documentation complies with healthcare regulations.
8. Problem Solving : Address any challenges or barriers that patients or the care team may face and work towards finding solutions.
9. Compliance and Quality Control : Ensure that care management processes comply with organizational policies, healthcare regulations, and standards of practice.
QUALIFICATIONS : The Lead Care Manager must have proven leadership experience, with the ability to supervise and mentor a team. Strong communication, organizational, and problem-solving skills. Knowledge of healthcare systems, insurance, and patient care protocols. Ability to handle sensitive patient information with confidentiality and professionalism. Strong interpersonal skills for managing and motivating a team. Strong understanding of healthcare regulations and compliance. Problem-solving and critical thinking abilities. Ability to handle sensitive and confidential information with discretion. Excellent communication skills, both written and verbal. Must be knowledgeable of current word processing and spreadsheet software, Microsoft Office, internet functions and knows how to use a computer. EDUCATION / EXPERIENCE : Masters prepared Registered Nurse (RN) preffered. Minimally, must be a Registered Nurse with 3+ years of experience in care coordination or healthcare management preferred. Previous roles as a care coordinator, case manager, or in similar positions. Experience with managing patient care plans, interacting with multidisciplinary teams, and handling complex cases.
LANGUAGE SKILLS : Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals; Ability to write routine reports and correspondence; Ability to speak effectively before groups of customers or employees of organizations; bilingual a plus.
REASONING ABILITY : Ability to define and solve practical problems, collect data, establish facts and draw valid conclusions. Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form
CERTIFICATES, LICENSES, REGISTRATIONS : Registered Nurse (RN) with a current Louisiana State Board of Nursing license. All candidates will be required to submit to a criminal background check and screening as part of preemployment.
