Position Overview :
The Hospice Social Worker provides comprehensive psychosocial support to terminally ill patients and their families, facilitating quality end-of-life care. This role focuses on emotional support, advance care planning, grief counseling, and practical assistance during one of life's most challenging transitions.
Key Responsibilities :
- Conduct initial and ongoing psychosocial assessments of hospice patients and families to identify emotional, spiritual, social, and practical needs.
- Provide compassionate counseling to patients and families addressing grief, loss, anxiety, depression, and end-of-life concerns.
- Facilitate advance care planning discussions including advance directives, healthcare proxies, and do-not-resuscitate orders.
- Offer anticipatory grief counseling and bereavement support to families before and after patient death.
- Coordinate practical assistance including financial resources, funeral planning, legal services, and community support.
- Serve as liaison between patient / family and the interdisciplinary hospice team including physicians, nurses, chaplains, and volunteers.
- Provide crisis intervention for emotional, family, or psychosocial emergencies.
- Assess and address caregiver stress, family dynamics, and support system needs.
- Connect families with bereavement services and support groups following patient death.
- Complete required documentation and participate in interdisciplinary team meetings.
- Make home visits, facility visits, or office visits based on patient location and needs.
- Provide education on hospice philosophy, services, and what to expect during the dying process.
Qualifications :
Master's degree in Social Work (MSW) from an accredited program required.State licensure (LMSW, LCSW, or equivalent) required.Experience in hospice, palliative care, medical social work, or grief counseling strongly preferred.Knowledge of end-of-life issues, grief and loss, and family systems.Cultural sensitivity and ability to work with diverse populations during vulnerable times.Strong clinical assessment and counseling skills with focus on person-centered, trauma-informed care.Excellent communication skills with ability to navigate difficult conversations with compassion.Ability to work independently and manage a flexible schedule including occasional on-call duties.Valid driver's license and reliable transportation for community-based visits.Emotional resilience and self-care practices to sustain work in end-of-life care.Working Conditions :
Combination of office work and travel to patient homes, hospitals, and care facilities.Flexible schedule may include occasional evenings, weekends, or on-call availability.Exposure to terminally ill patients and emotionally challenging situations.Physical requirements include ability to travel between locations and navigate various care environments.IMPORTANT :
