The principal responsibility of the Navigator RN is to efficiently facilitate the patient and family's navigation throughout the health care system. The Navigator supports patients from the initial referral through diagnosis and the development of a treatment plan, ensuring ongoing communication and seamless transitions during each phase of the patient's care journey. A team-based approach and collaboration are essential elements of the navigator, providing clinical insights to all members of the team. Activities encompass delivering disease-specific education and information, facilitating referral navigation, disseminating knowledge regarding available treatment options, accelerating patient access to diagnostic and therapeutic procedures, connecting patients with pertinent resources, offering continuous support to both patients and their families, and addressing barriers of care. The nurse navigator works in concert with the patient / family physician and clinical team. This position provides services throughout all phases of the care continuum, including prevention, screening, diagnosis, treatment, and end-of-life care.

Required Qualifications :

• Associate Degree - Graduate of accredited school of nursing
• Upon Hire : Texas Registered Nurse License
• Upon Hire : National Provider BLS - American Heart Association
• Within 1 year of hire date : National certification in area of practice
• 5 or more years of experience as a registered nurse
