Established Care : You get to establish and maintain long-term relationships with patients

The Role :

You will work within a high-quality, highly communicative / collaborative, and team-based architecture to oversee the comprehensive health and well-being of patients. Physicians will participate in regular huddles with their team to proactively identify health care issues and orchestrate a plan of care with the assistance of their team and the support of an entire organization.

What You'll Be Doing :

• Manage chronic and acute illness and injuries
• Establish and maintain long-term relationships with patients
• Provide well checks and house calls
• Be available after-hours by phone for patient communication and care
• Track and review normal physical development as well as psychological well-being
• Establish an outstanding relationships with and lead a team comprised of a Registered Nurse and Patient Care Administrator
• Create personalized reports for patients to help them and the team navigate their health goals and care
