Clinical Pharmacy Specialist

The Clinical Pharmacist Specialist Ambulatory Care / Operations works independently at the highest level of clinical practice within Ambulatory Care areas at the Ann Arbor VA Medical Center and the associated community-based outpatient clinics (CBOCs), as an integral part of the interdisciplinary healthcare team.

Clinical Practice :

• Promote the appropriate, economical, safe, and effective use of medication therapy through maintaining independent Ambulatory Care clinical pharmacy practice(s). The Clinical Pharmacy Specialist role will be in Primary Care / PACT supporting Canton and Flint. Expanded practice areas throughout the facility include :
• Anticoagulation Management Clinics
• Specialty Care Clinics
• Home-Based Primary Care Clinics
• Maintain collaborative and comprehensive medication therapy management clinics and services in designated areas of practice, under a scope of practice. This shall include physical assessment, ordering lab and diagnostic tests necessary to manage medication therapy, referring patients to appropriate health care providers as needed, performing patient triage functions, and independently initiating, adjusting, and discontinuing medications.

Travel is required within CBOCs for coverage of primary responsibilities at duty station and Canton CBOC for PACT face-to-face pharmacist provider - Veteran visits. Potential for coverage of Ann Arbor outpatient operations duties.

Work Schedule : Monday, Tuesday, Wednesday 7 : 00 am to 3 : 30 pm and Thursday, Friday 8 : 00 am to 4 : 30 pm
