The Clinical Pharmacist Specialist Ambulatory Care / Operations works independently at the highest level of clinical practice within Ambulatory Care areas at the Ann Arbor VA Medical Center and the associated community-based outpatient clinics (CBOCs), as an integral part of the interdisciplinary healthcare team.

Clinical Practice :
- Promote the appropriate, economical, safe, and effective use of medication therapy through maintaining independent Ambulatory Care clinical pharmacy practice(s). The Clinical Pharmacy Specialist role will be in Primary Care / PACT supporting Canton and Flint. Expanded practice areas throughout the facility include :
- Anticoagulation Management Clinics
- Specialty Care Clinics
- Home-Based Primary Care Clinics
- Maintain collaborative and comprehensive medication therapy management clinics and services in designated areas of practice, under a scope of practice. This shall include physical assessment, ordering lab and diagnostic tests necessary to manage medication therapy, referring patients to appropriate health care providers as needed, performing patient triage functions, and independently initiating, adjusting, and discontinuing medications.
Distributive Functions :
Perform the duties of a clinical pharmacist when working in the outpatient or inpatient pharmacy.Reviews all medication orders for appropriateness, drug selection, dosage, route of administration and the amount, prior to dispensing.Ensures all medication orders are entered into the electronic patient medical record.Fills all orders in a timely fashion; a systematic review of the patient's allergy history, drug-drug, drug-nutrient, drug-laboratory and drug-disease state interactions and assessment of the clinical relevance should be performed and dealt with appropriately.Reviews medication profiles and monitors for compliance or potential abuse; problems encountered shall be resolved.Selects, compounds, dispenses, and fills a range of drugs including extemporaneous preparations, special formulations, ophthalmics (subconjunctivals and intravitreal injections), narcotics, investigational drugs, oncolytic agents, etc., in both outpatient and inpatient areas. Adequate quality assurance procedures shall be followed as applicable.Maintains adequate drug stock, inspects drug storage areas, supervises prepacking and the operation of automated dispensing equipment.Supervises the activities of support personnel.Ensures drugs dispensed are completely and correctly labeled and packaged in full compliance with Federal rules and regulations.Understand medication use systems in the health system and their patient care impact.Educational Activities :
Supervise and train Pharmacy Practice residents and Doctor of Pharmacy students on rotation in area of practice.May serve as professional mentor to Pharmacy Practice residents.May serve as a mentor in the development and conduct of resident research projects including development of research questions, methodology, application to IRB, and assisting with presentation and manuscript.Scholarly Functions :
May actively participate in research activities.May assist in research efforts by other healthcare professionals.May provide professional and technical assistance to research investigators when needed.Administrative Function :
Assist the Ann Arbor Outpatient Pharmacy Supervisor and the Toledo Outpatient Supervisory Pharmacist in coordinating clinical pharmacist involvement in Ambulatory Care clinical pharmacy servicesReports adverse drug events (ADE), close calls, and medication errors in alignment with VA ADERS reporting program and the hospital Joint Patient Safety Reporting (JPSR) program respectively.When assigned, evaluates performance and competency of contract nursing homes. Makes recommendations for facility to achieve compliance in pharmacy related matters.Travel is required within CBOCs for coverage of primary responsibilities at duty station and Canton CBOC for PACT face-to-face pharmacist provider - Veteran visits. Potential for coverage of Ann Arbor outpatient operations duties.
Work Schedule : Monday, Tuesday, Wednesday 7 : 00 am to 3 : 30 pm and Thursday, Friday 8 : 00 am to 4 : 30 pm
