Practitioner must live within reasonable traveling distance to Danville, PA.

Program Highlights :

The AHCaH program utilizes remote patient monitoring, advanced technology, and interdisciplinary collaboration to deliver exceptional care to patients who meet hospital admission criteria.

Target Population & Conditions Treated :

• Med / Surgical level of care
• Congestive Heart Failure (CHF) exacerbation
• COPD exacerbation
• Urinary Tract Infections (UTIs)
• Other appropriate conditions

Your Role as an Advanced Practitioner (AP) :

As the core provider on the AHCaH team, you’ll work closely with patients and collaborate with a dynamic interdisciplinary care team under physician guidance. Key responsibilities include :
