Summary / TWC Summary JOB SUMMARY : The Community Health Education and Outreach Coordinator II is responsible for developing, implementing, and evaluating community health education and outreach programs to promote wellness and prevent disease. This role builds partnerships with community organizations, facilitates / supports health education and outreach event, analyzes and reports on data, and provides culturally relevant health information to the community. The Community Health Education and Outreach Coordinator participates in outreach and community engagement activities, assisting with community-facing initiatives, demonstrating independent actions necessary to provide competent and professional assistance to meet the needs of the team and community members.
DISTINGUISHING CHARACTERISTICS : The Community Health Education and Outreach Coordinator II is a highly collaborative, self-motivated, creative person that is adaptable to new processes with capacity to function independently under supervision. The Community Health Education and Outreach Coordinator is very knowledgeable of current publichealth trends.

Essential Duties Summary
Summary Community Health Education
- Develop and deliver community-based health education programs and presentations on topics such as chronic disease prevention, cancer prevention / screening, nutrition, Maternal Child health, Clinical research, mental health, reproductive health, and healthy lifestyle behaviors.
- Train and supervise community health workers or volunteers supporting outreach efforts
- Create educational materials, flyers, and digital content tailored to the needs of the target audience
Health promotions, Community Engagement, and Outreach
Initiates and maintains partnerships with local organizations, schools, faith-based groups, and public agencies to promote community engagementPlan and execute outreach campaigns in collaboration with community partners to raise awareness about available health services and resources.Represent the grant funded program initiatives at local coalitions, stakeholder meetings, and public forumsInitiates and coordinates community events, health fairs, screenings, and mobile clinics to engage diverse populationsMaintain accurate event calendar and communicate event details with the teamAdministrative and Financial
Collect data, analyze data, prepare reports, and evaluate program effectiveness to ensure continuous improvement.Answers incoming telephones, troubleshoots and directs calls and requests to appropriate individuals while evaluating, documenting, and managing their needs.Interacts with peers, staff, and physicians in a supportive and respectful manner.
