Position : Physical Therapist - Home Health

Facility : Sheldon Medical Center

Location : 118 N 7th Ave, Sheldon, IA 51201, USA

Shift : Day

Schedule : PRN

Salary Range : $37.50 - $62.00

Job Summary :

As a Physical Therapist, you will :

• Plan, organize, and deliver physical therapy programs to enhance patient mobility, relieve pain, and prevent deformities.
• Conduct thorough examinations and evaluations for patients affected by injury or disease.
• Utilize current research and best practices to deliver effective treatments tailored to individual needs.
• Educate patients and their families about treatment plans, schedules, and at-home exercises.
• Document patient information diligently and in accordance with departmental guidelines and regulatory requirements.
• Demonstrate flexibility in work schedules and actively promote the department and Sanford Health.
• Engage with a variety of individuals outside the department, including providers and case managers.
• Possibly supervise assistants, aides, and students in providing care and treatment.
• Participate in ongoing education and training for peers in relevant specializations.

Based on facility needs, you may focus on delivering care in post-acute, long-term care, and home health environments.
