Purpose : Following a medically prescribed physical therapy program, this position evaluates, treats and educates pediatric, adolescent, adult or geriatric patients who have disabilities, traumatic injuries, pain, disease or any dysfunction. The Physical Therapist helps patients reach maximum performance and assume a place in their community.

Essential Responsibilities

1. Provides direct patient care services to ensure the effectiveness of physical therapy programs while adhering to safety policies and procedures.

2. Evaluates patients and establishes an effective treatment plan to achieve set goals.

3. Provides physician prescribed physical therapy treatment to all patients within the assigned caseload, to return the patient to optimum and productive levels within the patient's capabilities.

4. Prioritizes caseloads and available treatment times to provide for maximum efficiency.

5. Improves or minimizes residual physical disabilities of the patient by utilizing correct modalities of treatment and suggests variations.

6. Recognizes contraindications to continuing with scheduled treatment and consuls with responsible Physician.

7. Involves family or caregivers in treatment plan and discharge planning.

8. Interprets the appropriate information needed to identify each patient's requirements relative to age-specific needs.

9. Performs appropriate documentation to effectively communicate the patients' functional status to Physicians, insurance companies, the patient their caregivers.

10. Consults with hospital personnel to provide for quality patient care and cost-efficient programming and provides patient / caregiver education for home exercise programs and to prepare for discharge.

Job Qualifications :

• Education : Graduated from an accredited physical therapy school with a B.S. or M.S. degree.
• Experience : New graduates welcome
• License : New York State license or eligibility with temporary permit.
