About This Opportunity :
The Nurse Navigator in Outpatient Behavioral Health Services is essential in connecting individuals with the mental health care they deserve. This dynamic role facilitates seamless patient access to treatment, working collaboratively with a multidisciplinary team that includes psychiatrists, therapists, social workers, and community organizations. Your efforts will ensure patients receive comprehensive, timely, and individualized care tailored to their needs. By engaging in outreach, identifying patient needs, facilitating referrals, and providing ongoing support, the Nurse Navigator will significantly enhance patient adherence to treatment and improve overall wellness in our community.
Key Responsibilities :
- Guide patients through the outpatient behavioral health services ensuring timely appointments, follow-ups, and continuity of care.
- Serve as the primary contact for patients and families, answering inquiries and clarifying treatment plans.
- Conduct outreach to identify needs and risk factors related to medical, behavioral, and social health.
- Facilitate referrals to appropriate behavioral health services and community support programs.
- Monitor and track patient progress, ensuring adherence to treatment plans and enhancing engagement through follow-ups.
- Collaborate with the clinical team to create coordinated care plans for patients.
- Educate patients and families on behavioral health conditions, treatment options, and coping strategies.
- Maintain accurate documentation in electronic health records (EHR) and contribute to quality improvement initiatives.
- Identify high-risk patients and facilitate timely interventions to prevent crises or non-adherence to treatment.
Qualifications :
Bachelor's degree in nursing is required; a Master's degree is preferred.2-4 years of clinical nursing experience in behavioral health, mental health, or outpatient care is necessary.Experience with patient education, care coordination, or case management in a healthcare context is a must.Familiarity with managing complex behavioral health needs, including mental health and substance use disorders.Proven ability to collaborate with multidisciplinary teams.Experience with electronic health records (EHR) for patient tracking and documentation is required.Prior experience in patient advocacy or community health initiatives is a plus.Licensure and Certifications :
Active, unencumbered Registered Nurse (RN) license in the state of practice is essential.Current Basic Life Support (BLS) certification is required.Advanced Cardiac Life Support (ACLS) certification is preferred.Certification in psychiatric or behavioral health nursing (PMH-BC) or case management (CCM, ACM) is advantageous.Experience or certification in care coordination, patient navigation, or behavioral health case management is desirable.This position offers a competitive salary range of : USD $89,065.00 - USD $162,801.00 / Yr.
