The Community Care Nurse Practitioner (NP) is a professional nurse with advanced clinical knowledge and competence to provide professional assessment, coordination, and planning of multiple health care services. S / he is accountable for the management and evaluation of patients under his / her care. S / he holds clinical privileges and practices in collaboration with the Community Care team. Clinical core privileges will include : the clinical and administrative evaluation and management of Community Care / non-VA care consults for approval per local and national guidelines and processes; the management, treatment, and ordering, performing, and interpreting labs, radiography, and other diagnostic tests for adult patients who are traveling between VA facilities. The Community Care NP will report directly to the Chief of Administrative Medicine Service.
The incumbent is responsible for the clinical and administrative oversight of consults and additional services processed per Community Care guidelines. S / he demonstrates sound decision making skills in review of patients to determine eligibility for non-VA care services. The incumbent assists in the coordination of care with non-VA providers, functioning as a liaison between the Veteran, SAVAHCS, and community medical providers to include hospitals, medical offices, and private providers. S / he uses knowledge of the resources and specialty care available within the VA to coordinate comprehensive care for patient's non-VA care needs. S / he provides guidance to all Community Care staff and assists in the training of Community Care staff or other stakeholders in the use of Community Care Programs.
Additional Community Care NP duties include : Reviews, authorizes, and completes Community Care consults; reviews consults for completeness and accuracy; completes medical reviews on secondary authorization requests; provides clarification of requested non-VA care and expedites processing of requests; reviews and assists with resolution of complex Community Care medical cases generated by Congressional or patient inquiry; verifies proper jurisdictions, urgency, appropriateness, and availability of care within the VA on all non-VA care referrals; serves as a liaison and works closely with community hospital and provider representatives to ensure maximum interchange of information necessary to accomplish Community Care's program mission; schedules and conducts regular meetings with community partners; participate in on-going local and national Community Care program office meetings and trainings; provides training to VA providers and enforces compliance to Community Care program regulations and processes; works with Community Care nurse managers, administrative supervisors, clinical staff, and administrative staff to ensure completion of programmatic processes, acquisition of national program metrics, and implementation of system and process improvements; works with the Third Party Administrator staff on complex case and care issues.
The incumbent provides indirect care to traveling Veterans, working with the Traveling Veteran coordinator (TVC) Registered Nurse to afford provider level oversight and input for the coordination of care for Veterans who are assigned to VA medical centers other than SAVAHCS and who spend part of the year in the SAVAHCS catchment area. In-direct care of traveling Veterans includes : reviewing care requests from other VA medical centers; reviewing and / or ordering consults, diagnostic tests, medical supplies, and medications based on input from the Veteran's primary VA medical center; coordinating with SAVAHCS Primary Care and Specialty Care services to provide needed care; assisting the TVC Registered Nurse as necessary in coordination of care of Veterans primarily assigned to SAVAHCS but who are traveling to other VA medical centers.

The Accreditation Commission for Education in Nursing (ACEN) or
NOTE : Grandfathering Provision - All persons currently employed in VHA in 0610 series and an APN (NP) assignment that are also performing the duties as described in the qualification standard on the effective date of the standard (1 / 29 / 2024) are considered to have met all qualification requirements for the grade held including positive education and licensure / certification.
The following Scope, Education and Experience, and Dimension criteria must be met to qualify for Advanced Practice Nurse (Nurse Practitioner) II :
Exception to Licensure for Graduate Nurse Technicians. Candidates who otherwise meet the basic requirements, but do not possess the required licensure and / or certification, may be appointed at the entry step of the Nurse II, as a Graduate Nurse Technician - Nurse Practitioner on a temporary appointment up to any period not-to-exceed one year and a day under the authority of 38 U.S.C. § 405(c)(2)(B).
Clinical Practice. Delivers evidence-based health care, demonstrating proficiency in the ability to manage a patient panel, population, or program.
The following Scope, Education and Experience, and Dimension criteria must be met to qualify for Advanced Practice Nurse (Nurse Practitioner) III :
