Responsibilities
As a Supportive Care / Palliative Care Nurse Practitioner (SPCNP), you will collaborate with HarmonyCares Medical Group staff to provide comprehensive and quality palliative care services to patients under the guidance of the supervising Regional Chief Medical Officer. Your responsibilities will include conducting patient visits—both new and existing—for palliative care and coordinating care with the designated palliative care team. You’ll visit patients at home, in facilities, and hospitals, making independent decisions regarding their healthcare concerns and goals of care as needed.
