Care Management Support Coordinator II
Pay : $17 / hr
Schedule : Monday-Friday, 8 : 00 AM - 5 : 00 PM
Location : Must reside in Hidalgo SDA, TX (McAllen, Brownsville, Weslaco, Mercedes, Alamo, Rio Grande, Roma)
Position Purpose :
Provide administrative care management support including outreach, inbound call handling, and scheduling services. Serve as a key point of contact for members, providers, and internal staff, resolving issues and maintaining member records in compliance with state and regulatory guidelines.
Responsibilities Conduct outreach calls to members to support care plan steps, answer questions, and connect to health plan or community resources.
Assist members with scheduling services, MD appointments, and transportation arrangements.
Perform provider searches and follow up on pending services within required timelines (e.g., 28-day follow-up).
Coordinate with social workers for medical necessity determinations.
Manage mail-outs, inventory, and ordering of collateral resources.
Support disaster response calls, provider termination / closure notifications, and other outreach campaigns.
Maintain non-clinical member records in accordance with policies and regulations.
Make referrals for Social Determinants of Health (SDOH) needs as appropriate.
Complete other administrative duties as assigned.
Requirements
Education : High School diploma or GED required.
Experience : 1-2 years of related administrative or healthcare experience.
Strong computer skills, with proficiency in Microsoft Word, Excel, and PowerPoint.
Knowledge of medical terminology preferred.
Preferred : Bilingual (English / Spanish).
