Nurse Case Manager

Coordinates the multidisciplinary activities related to maintaining continuity of care from the hospital to the home setting. Responsible for all discharge planning activities, including team conferences and unit-based Multidisciplinary Patient-Focused Rounds. Reviews Medical Record for appropriateness of care while monitoring length of stay in a collaborative effort with the Medical Staff, Nursing, Ancillary Departments and the Business Office.

Education : BSN / BSW preferred

1.Licensure / Registration / Certification : NYS Licensure as RN required; LMSW / LCSW preferred; Case management certification preferred.

2.Work Experience : 3 years RN experience in an acute care setting; Leadership experience preferred; Utilization review, discharge planning or case management experience preferred.

3.Physical Requirements : Ability to work under stress in a busy department; walking, bending and stretching. Sedentary activities include committee meeting, chart reviews, telephone inquiries, follow-up calls, interviewing, computer data entry and word processing and data trending / analysis.

4.Work Environment / Occupational Hazards : Exposure to computer monitors, patients' conditions, contagious diseases and some unpleasant sights and smells.

5.Job Relationships : Works effectively with healthcare team members, administrative personnel and medical staff; Responsible to Administrative Director of Clinical Resource Management and Manager of Case Management. knowledge and expertise; knowledge of regulatory standards and current health care and act as a liaison between medical, professional, clinical and support staff; problem change by utilizing concepts of change theory and team building.
