Remote Licensed MSW
We are looking for a Remote Licensed MSW to join our team, covering Grand Rapids, MI.
Join a mission-driven team that is redefining how care is delivered to patients facing serious illness. As a Social Worker in our Advanced Illness Management (AIM) program, you will provide psychosocial support, guide patients and families through complex care decisions, and help navigate the emotional, spiritual, and social needs that accompany serious illness.
You will work collaboratively with physicians, interdisciplinary teams (IDT), and community partners to deliver compassionate, patient-centered care aligned with individual values and goals.
Remote position
Licensure in Michigan
Monday-Friday, 8am-5pm
Essential functions of position :
- Assess emotional, psychosocial, and mental health needs of patients and families facing serious illness
- Identify signs of distress, anticipatory grief, and coping challenges, offering targeted interventions
- Guide patients and families through goals-of-care conversations and the completion of Advance Directives or Do Not Resuscitate (DNR) orders
- Develop and maintain individualized care plans in collaboration with AIM, IDT, PCPs, and external providers
- Identify, coordinate, and advocate for access to community-based resources and support systems
- Respect and integrate cultural, spiritual, and family dynamics into care delivery
- Maintain thorough and timely documentation of visits, interventions, and care outcomes
- Adhere to HIPAA, Medicare regulations, company policies, and ethical social work standards
- Maintain updated lists of community resources and stay informed of changes in services or regulations
- Provide support through virtual visits from a secure home-based workspace
About You
Education and licensure :
Master's degree in Social Work (MSW)Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW)Must hold or be able to obtain multistate licensure - Licensure in MichiganExperience requirements :
Minimum 2 years of skilled social work experienceStrong knowledge of health care systems and care options across multiple levels of careExperience in hospice, palliative care, geriatrics, or oncology preferredDemonstrated ability to deliver high-quality, compassionate support to patients and familiesSpecialized knowledge and skills :
Skilled in crisis intervention, psychosocial assessment, and advance care planningExcellent communication, organization, and time management skillsCollaborative and customer-service focusedCompetence in Microsoft Word, Excel, and OutlookTraits and aptitudes :
