Registered Nurse Case Manager
What You Must Have :
- Current State Registered Nurse License
- Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance
- 1-3 years previous Home Health Care / Long Term Care experience, Hospice preferred
