job_description.job_card.job_descriptionManage a caseload of home health patients, coordinating and delivering personalized care plans. Conduct initial and follow-up assessments to identify the physical, psychosocial, and environmental needs of patients. Develop, implement, and evaluate individualized care plans in collaboration with interdisciplinary teams. Maintain clear and timely communication with the Clinical Supervisor, physicians, and other team members regarding any changes in patient status or care needs. Ensure patient confidentiality and compliance with HIPAA regulations. Document all care and communication according to policy and procedure. Current, unrestricted RN license in Massachusetts. Minimum of 1 year of experience in a clinical care setting. Prior home health or hospice experience is preferred. Reliable vehicle, valid driver's license, and current auto insurance. Ability to travel 50% within the service area. Associate's or Bachelor's Degree in Nursing preferred. Salary Range : $90,000 - $100,000 per year
