Role Overview
As a hospice aide, you will deliver vital personal care to our patients under the guidance of a registered nurse. This is a part-time position with the potential to transition to full-time. Shifts include 5-10 for tuck-ins and 7-2 for morning care.

Key Responsibilities
Document care tasks according to organizational standards.Notify the Case Manager of any deviations from the Plan of Care.Utilize medical equipment safely, including hoyer lifts and sit-to-stand devices.Follow the Hospice Aide Plan of Care provided by the RN Case Manager.Collaborate with team members and actively participate in patient care discussions.Monitor and report any changes to a patient’s skin condition to the Case Manager.Assist with mobility support, transfer, and range of motion exercises as outlined in care plans.Engage in Quality Assurance and Performance Improvement activities.Qualifications
Must hold an Active CNA License in Utah.A minimum of one year of experience as a CNA, hospice aide, or home health aide is required.Applicants should be at least 18 years of age.Must possess a valid driver’s license and have access to a reliable, insured vehicle.Excellent communication skills, both written and verbal, are essential.Able to work effectively both as part of a team and independently.Join us in making a difference in the lives of our patients and their families!
