Licensed Behavioral Health Case Manager

A company is looking for a Behavioral Health Case Manager. Key Responsibilities Screen and assess patients for common mental health disorders and facilitate patient engagement Provide brief behavioral interventions and track treatment response and patient progress Collaborate with a multidisciplinary team to develop personalized treatment plans and coordinate care Required Qualifications Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), Licensed Professional Counselor (LPC), or equivalent Minimum 1-year of direct clinical experience working with children and adolescents Experience in delivering brief psychosocial evidence-based treatments Experience with care coordination in a team-based healthcare setting Working knowledge of differential diagnosis of common mental health disorders
