Case Manager - IICAPS

Bridges Healthcare, Inc. is seeking a Case Manager for our IICAPS Program. $21.76 - $27.96 per hour, commensurate of experience. Full time / 35 hour work week. Benefits for the position include :

Purpose and Position Overview for IICAP'S Case Manager

Provide home-based clinical case management to children with psychiatric illnesses and their families.

Duties and Responsibilities

  • Provide home-based case management services to children and their families.
  • Co-facilitate and family sessions
  • Provide crisis intervention
  • Maintain client records.
  • Obtain and monitor insurance authorizations.
  • Serve as liaison to other service providers.
  • Perform any other department or agency related duties or special projects as directed by supervisor.

    • Qualifications and Competencies

  • Bachelor's degree required, with major in human services preferred.
  • In home experience preferred.
  • Prior experience providing services to children and families who have mental health disorders.
  • Crisis intervention skills.
  • Experience working and collaborating with community providers.
  • Ability to work a flexible schedule.
  • Sensitivity to diverse cultures and language competencies.
  • Knowledge of behavioral health issues and disorders for children and families
  • Good communication and documentation skills.
  • Computer literacy.
  • Ability to prioritize and organize schedules / demands.
  • CPR / First Aid certified or willing to become certified.
  • Valid State of Connecticut driver's license and access to reliable transportation.
