Full-Time Hospice RN Case Manager

Location : San Antonio, TX & Surrounding Areas

Position Overview :

We are seeking a Full-Time Hospice RN Case Manager to join our dedicated team. This role requires strong clinical judgment, excellent communication skills, and a heart for hospice care. The RN Case Manager is responsible for managing a caseload of patients, coordinating care with the interdisciplinary group (IDG), and providing both direct patient care and supportive guidance to families.

Key Responsibilities :

  • Manage and oversee the care of assigned hospice patients, ensuring individualized care plans are followed.
  • Perform comprehensive patient assessments, symptom management, and education for patients and families.
  • Attend and actively participate in Interdisciplinary Group (IDG) meetings.
  • Coordinate with physicians, social workers, chaplains, and other team members to ensure seamless care delivery.
  • Maintain accurate and timely documentation in accordance with hospice and regulatory requirements.
  • Participate in a rotating on-call schedule, with at least one full week of on-call duties per month included in salary .
  • Travel throughout San Antonio and surrounding areas to visit patients in their homes, facilities, or other care settings.
  • Qualifications :

  • Current RN license in the state of Texas (in good standing).
  • Minimum of 1 year of hospice or related nursing experience preferred.
  • Strong assessment, clinical, and problem-solving skills.
  • Comfortable with travel across San Antonio and surrounding communities.
  • Excellent organizational and communication skills.
  • Valid drivers license, reliable transportation, and proof of insurance.
