About this Job :
This role involves providing vital social work services to patients within MedStar Health Homecare and Montgomery County clients through MHHC Care Partners. As a Social Worker, you will offer counseling and connect patients with community resources as needed, adhering to home health regulations and county contract requirements. You will perform assessments and address social issues that may affect patient rehabilitation and compliance, while supporting the health team in understanding the psychological and emotional factors impacting patient health.
Key Responsibilities :
- Support the achievement of established goals, ensuring adherence to department policies, quality standards, and safety regulations. Complete mandatory training and comply with all relevant regulations.
- Accurately document care provision in accordance with state and federal regulations, and communicate effectively with other healthcare providers.
- Collaborate with healthcare team members, patients, and families to develop and execute individualized social care plans based on assessed needs, continually reassessing and modifying as necessary.
- Foster positive working relationships with other departments and participate in multidisciplinary quality improvement initiatives. Engage in community outreach to enhance service provision.
- Provide actions to meet patient needs, including resource linkage, short-term counseling, and assistance with nursing home placements. Support staff members regarding the social and emotional needs of their patients.
Minimum Qualifications :
Education :
Master's degree in Social Work from a Council on Social Work accredited institution required.Experience :
1-2 years of social work experience in a healthcare setting required; home care experience preferred.Licenses and Certifications :
Licensed Social Worker (LGSW, LCSW, LCSW-C, LISW, or LICSW) required upon hire.Valid state driver’s license in good standing and access to a dependable vehicle required upon hire.CPR certification (AHA or American Red Cross) required within 90 days.Other unit / specialty certifications may vary by department.Knowledge, Skills, and Abilities :
Strong clinical skills.Excellent verbal and written communication skills.Basic computer skills preferred.This position offers a competitive salary range of : USD $61,838.00 - USD $111,259.00 / Yr.
