Occupational Therapist Part Time for Home Health

Occupational Therapist Job Summary :

Deer Meadows Home Health is seeking a dedicated and compassionate Occupational Therapist to join our Home Health team on a part-time basis. As an integral member of our interdisciplinary team, you will play a vital role in helping patients recover and achieve their maximum potential in the comfort of their own homes.

Occupational Therapist Responsibilities :

Conduct thorough assessments of patients' physical and cognitive abilities, identifying areas of strength and weakness

Develop and implement individualized occupational therapy plans to address patients' specific needs and goals

Collaborate with patients, families, and healthcare providers to ensure seamless transitions and optimal outcomes

Provide education and training to patients and caregivers on techniques and strategies for improving daily functioning and independence

Document patient progress and maintain accurate records

Participate in case conferences and team meetings to ensure comprehensive care

Occupational Therapist Requirements :

Current licensure as an Occupational Therapist in the state of Pennsylvania

Prior experience in treating in Home Health

Ability to travel to patient homes and work independently

Must have own reliable transportation; travel reimbursements provided

Strong communication and interpersonal skills

Compassionate and patient-centered approach to care

Ability to work effectively in a fast-paced environment
