RN Care Manager

A company is looking for a Care Manager Transitions of Care. Key Responsibilities Evaluate member needs through post-discharge assessments and medication reconciliation Develop care / service plans in collaboration with healthcare teams to support member transitions Facilitate communication and services between medical providers and members to address care needs Required Qualifications, Training, and Education Master's degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing 2-4 years of related experience in care management Active RN license required Experience in coordinating care across various healthcare settings Knowledge of state, federal, and third-party payer regulations
