About The Position
Job Title : Case Manager I Profession : Registered Nurse Specialty : Case Management Duration : 13 weeks Shift : Days Hours per Shift : 8 hours (8 : 00 AM - 4 : 30 PM) Experience : Minimum of 2 years in case management License : Current and valid license to practice as a Registered Nurse in the State of Michigan Certifications : BSN degree Must-Have : - Minimum of 2 years of case management experience - Current active RN licensure - Bachelor of Science in Nursing Description : The purpose of the Case Manager I position is to support the physician and interdisciplinary team in facilitating patient care. The underlying objective is to enhance the quality of clinical outcomes and patient satisfaction while managing the cost of care. The role integrates and coordinates utilization management, patient education, care facilitation, and discharge planning functions. The Case Manager I is accountable for a designated patient caseload and plans effectively to meet patient needs. This position manages the length of stay and promotes efficient utilization of resources. Principal Duties And Responsibilities : Facilitate the precertification and payer authorization processes. Facilitate the collaborative management of patient care across the continuum, intervening as necessary. Remove barriers to timely and efficient care delivery and reimbursement. Apply process improvement methodologies in evaluating outcomes of care. Support and coach clinical documentation efforts as a clinical resource for coders. Ensure that documentation accurately reflects severity of illness and intensity of service. Coordinate communication with physicians. Demonstrate knowledge and skills necessary to provide care appropriate to the patient served. Demonstrate knowledge of the principles of growth and development as it relates to different life cycles. Required Skills & Experience : Possess interpersonal communication and negotiation skills. Possess analytical, data management, and PC skills. Exhibit organizational and time management skills with the capacity to prioritize multiple tasks. Work independently and exercise sound judgment in interactions with various stakeholders. Preferred Skills & Experience : Minimum of three years of clinical experience is preferred. Current working knowledge of discharge planning, utilization management, and case management is preferred.
