Inpatient Palliative Care Practice Provider

Job Summary :

Inpatient Palliative Care Practice Provider will provide Full Time care to patients who require specialized Palliative Care Consultation . This position is located at SUNY Upstate Downtown with rotation to the Community Campus .

Responsibilities include but not limited to :

• Consultative services to assure identification and communication of patient goals of care.
• Symptom management recommendation and care.
• Providing acute and chronic pain management through appropriate assessment.
• Evaluation and treatment, education, and guidance on the principles of palliative care.
• Institutional leadership in all process improvement activities related to palliative care.
• Maintaining outstanding relationships with families, referring providers and all members of the health care team.

SUNY Upstate APP's care for patients, families and the community through assessment, planning, prescribing and evaluation interventions.
