Job Summary
We are looking for a licensed Speech Therapist to provide comprehensive speech, language, and swallowing therapy to patients in a home health setting. The ideal candidate will have strong clinical skills, a passion for patient care, and the ability to work independently while collaborating with a multidisciplinary team.
Responsibilities
Conduct assessments to evaluate patients’ speech, language, cognitive-communication, and swallowing abilities.
Develop and implement individualized treatment plans based on patient needs and goals.
Provide therapy to improve communication skills, including articulation, fluency, voice, and language comprehension / expression.
Address swallowing disorders (dysphagia) through therapeutic interventions and patient / family education.
Document patient progress, maintain accurate records, and communicate effectively with healthcare team members.
Educate patients and families on strategies to support communication and swallowing goals.
Adhere to all state and federal regulations, as well as company policies and procedures.
Qualifications
Master’s degree in Speech-Language Pathology from an accredited program.
Current state licensure as a Speech-Language Pathologist (SLP) in [State].
Certificate of Clinical Competence (CCC-SLP) from the American Speech-Language-Hearing Association (ASHA) preferred.
Minimum of 1 year of experience in speech therapy; home health experience is a plus.
Strong interpersonal and communication skills with the ability to build rapport with patients and families.
Ability to work independently, manage time effectively, and travel to patients’ homes.
Valid driver’s license, reliable transportation, and auto insurance.
Proficiency in electronic medical record (EMR) systems.
