The Social Worker Home Health and Hospice is a member of the Interdisciplinary Health team and is responsible for providing social work services in the area of Hospice care. The social worker may provide direct care, teach, supervise and consult. The social worker provides service under written orders of the patient's physician and the Team's Plan of Care and under continuing medical direction.
Job Duties and Responsibilities :
Physical and Sensory Requirements :
Medium Work : Exertional activity Occasionally lift and / or carry 50 lbs. Frequently lift and / or carry 25 lbs. Stand and / or walk at least 6 hours in an 8-hour work day.
Nonexertional activity Frequent stooping (bending at the waist) and crouching (stooping and bending at the knee). Frequent Handling (grasping). Occasional Fingering (fine manipulation).
Important : The functional capacity to perform medium work includes the functional capacity to perform sedentary and light work.
Education : Master's Degree required. Current, acceptable professional license to practice Social Work in the Commonwealth of Pennsylvania preferred.
Training and Experience : Two (2) years of social work experience in health care setting prefer home health, oncology or hospice experience preferred.
