As part of our mission-driven interdisciplinary team (IDT) our Primary Care Providers (PCPs) care for fewer patients allowing them to build deeper more long-term relationships than in a traditional healthcare model allowing them to see the impact of their care every day. Our value-based PACE program allows our PCPs to provide truly comprehensive holistic care so they can focus on what really matters : their patients.

Competitive compensation of $165000 - $203000 with quarterly bonus
On the day-to-day you will :
Assist in the recruitment training precepting and onboarding of new PCPs and preceptors
Act as the subject matter expert on primary care workflows
Identify opportunities for process improvements and implement changes to enhance patient care and clinic efficiency
Collaborate with other leaders to create and update PCP trainings
Conduct a weekly review of KPIs as well as support PCPs in managing and understanding their KPIs
Oversee clinical pods in conjunction with the Medical Director / Associate Medical Director to make clinical decisions
Assist with preparing for and participating in center audits and inspections
Job requirements include :
Graduate of an Accredited School of Nursing or Physician Assistant program
Active and unrestricted CA state license or Certification as a Nurse Practitioner or Physician Assistant with Prescriptive Authority
Active Drug Enforcement Administration (DEA) Registration
Must be currently enrolled or eligible for enrollment to provide services to Medicare and Medi-Cal participants
