PHYSICAL THERAPIST ASSISTANTS - (PT) PART-TIME / PRN

Home Care Agency is looking for a Physical Therapist Assistant to join our home care agency.

The Physical Therapist Assistant is responsible for providing rehabilitative services to clients in their homes under the direction and supervision of a licensed Physical Therapist. The Physical Therapist Assistant assists clients improve mobility, therapeutic exercises and the use of self-help devices.

Responsibilities :

• Implement treatment plans as directed by the licensed Physical Therapist
• As directed by licensed Physical Therapist, performs routine strengthening and therapeutic exercises and assists the client with the use of self-help devices
• Accurately document all client care provided, including progress notes and observations
• Assist the licensed Physical Therapist with preparing reports, discharge summaries, and update care plan.
• Work to identify and solve clients' concerns
• Teaches, supervises, and counsels the client and family in the physical therapy program.
• Investigates and resolves patient / family member concerns regarding patient care.
