Home Health Physical Therapist

Our expanding healthcare organization is looking for a compassionate and skilled Home Health Physical Therapist to provide in-home patient care across Cook, Will, DuPage, Lake, McHenry, and Kane counties.

Job Overview

The Physical Therapist will evaluate patients, develop customized treatment plans, and implement therapies to improve mobility and overall function.

Qualifications

  • Active Physical Therapy (PT) license in [Your State]
  • At least one year of experience in home health or outpatient care (preferred)
  • Ability to create and execute personalized treatment plans
  • Strong documentation and EMR proficiency
  • Excellent communication and interpersonal skills

Key Responsibilities

  • Assess patient conditions and design effective treatment plans
  • Utilize exercises, physical modalities, and manual therapy to enhance mobility and function
  • Maintain detailed records of treatments, coding, and billing
  • Collaborate with other healthcare professionals to optimize patient outcomes
  • Ensure accurate and timely electronic documentation
  • Provide high-quality care, consistently demonstrating advanced evaluation and therapy techniques
  • Contribute to cost-saving initiatives within the department
  • Actively participate in performance improvement efforts
  • Foster strong communication between physical therapists, assistants, and physicians

    • Work Environment

  • Requires travel for in-home patient visits
  • Occasional work at the main office in Skokie, IL

    • Supervision & Reporting

    Reports to : Director of Patient Care Services / Agency Supervisor

  • May supervise : Licensed Occupational Therapists, Home Health Aides

    • Physical Demands

  • Must be able to stand, lift, bend, and perform physical tasks for extended periods
  • Ability to support patient mobility and perform heavy lifting as needed

  • Per-Visit Rate : Starting at $65.00 per visit

  • Graduate of an accredited Physical Therapy program
  • PT License (Required)
