Hospice case management position, Servicing North Milwaukee and the counties of Washington, Ozaukee, Waukesha, and Jefferson

urse must be comfortable providing end of life care and planning, wound assessments, care coordination, pain / symptom management, and able to triage appropriately. Patients will be ADULT ONLY and end of life. 1 : 1 patient ratios. Support on contract consists of other Case Managers, RNs, and a Resource Nurse. Team uses EPIC Remote Client - someone with previous experience and able to hit the ground running is highly preferred. Up to 1 week of orientation is available, based on comfort level. Sapphire IV pumps for patients. Any color scrub OR business casual is acceptable.
