About the Role

The Medical Assistant for Virtual Care Coordination provides clinical and administrative support to ensure timely and effective follow-up care after virtual visits. This includes managing outbound referrals, lab and imaging orders, prescriptions, and supporting behavioral health and chronic care reporting. This position plays a key role in helping patients complete next steps in their care plans while supporting providers and internal teams with accurate, timely coordination.

Key Responsibilities

  • Medical Assistant I :

o Process outbound referrals and prescription orders based on provider instructions

o Coordinate laboratory, imaging, and other ancillary services as ordered

o Handle incoming prescription refill requests and route to providers for review

o Track and close open follow-up items and patient care tasks

o Support reporting and tracking of behavioral health screenings (PHQ-9, GAD-7) and Chronic Care Management (CCM) activities

o Triage and escalate clinical concerns to appropriate providers or teams

o Submit and follow up on prior authorization requests

o Respond to and process medical records requests in accordance with HIPAA

o Manage Creyos cognitive testing : send to patients, monitor for completion, upload results

o Escalate incomplete provider notes, tasks, or orders to Provider Relations and Services Operations

o Other duties as assigned

  • Medical Assistant II :
  • o All Medical Assistant I responsibilities

    o Serve as a liaison for training new team members

    o Assist with problem solving and resolving patient concerns

  • Medical Assistant III :
  • o All Medical Assistant I & II responsibilities

    o Leads and is accountable for new team member training and development for 30-, 60-, & 90-day expectations

    o Support Clinical Operations Managers and Directors in various responsibilities

    o Serves as a team lead to disseminate updates on a routine basis.

    o Foster overall team productivity

    o Set team goals for daily, weekly, & monthly objectives

    Qualifications

  • Skills
  • o Bilingual (English / Spanish) a plus

    o Knowledge of medical terminology

    o Strong organizational skills

    o Able to follow up on orders and tasks

    o Good written communication

    o Comfortable reviewing medical notes and tasks

    o Able to recognize when to escalate clinical issues

    o Reliable with completing follow-up steps

    o Works well independently

  • Experience
  • o Medical Assistant I :

    Minimum 1 year experience in a healthcare setting supporting clinical workflows

    Familiarity with prescription processing, referrals, prior authorizations, and care coordination

    Previous experience utilizing an EMR / EHR a plus

    Telemedicine experience a plus

  • Education
  • o High school diploma or GED required

    o Active Medical Assistant certification (CMA, RMA, or equivalent) preferred

    Physical Requirements

  • Prolonged periods of desk work and computer usage.
  • Ability to lift up to 15 pounds.
