Registered Nurse (Veterans Health Education Coordinator)

The Eastern Colorado VA Health Care System is recruiting for an Registered Nurse to serve as the Veterans Health Education Coordinator. The RN Delivers a fundamental understanding of Veteran-centered health education and adult learning principles and is a lifelong learner that can practice with flexibility to adapt or change learning strategies based on the assessment of health education and learning needs within the healthcare organization.

The Registered Nurse Veterans Health Education Coordinator delivers a fundamental understanding of Veteran-centered health education and adult learning principles and is a lifelong learner that can practice with flexibility to adapt or change learning strategies based on the assessment of health education and learning needs within the healthcare organization.

Responsibilities include, but not limited to :

• Assesses, plans, implements, and evaluates educational needs to facilitate learner development and skill training to help Veterans treat and prevent health conditions and optimize their whole health, well-being, and quality of life.
• Utilizes innovative health education teaching strategies, assessment skills, and evaluation of outcomes.
• Assumes responsibility for the coordination of education focused on patient education, self-management, healthy living, and customer satisfaction throughout the healthcare continuum.
• Supports a coordinated approach to planning, delivery, and evaluating evidence-based Veteran-centered health education programs, services, and resources. Follows procedures per established policies and guidelines.
• Improves Veteran / patient driven care by resolving experience issues such as coordination of care, timeliness or process issues, common themes of dissatisfaction, or environment of care concerns for the immediate practice setting.
• Serves as an advocate for patients, families, and caregivers, improving outcomes for the immediate practice setting.
• All other assigned duties.

Pay : Competitive salary, regular salary increases, potential for performance awards
