Licensed Vocational Nurse / LVN

TexMed Home Health specializes in offering exceptional care to each client in all stages of life. We provide services to clients in private homes, assisted living communities, hospitals, skilled nursing facilities, and rehabilitation centers. We are actively looking for compassionate team members who desire to help our clients remain independent and have their best life now!

The primary function is to administer skilled nursing care, under the supervision of a registered nurse, for clients of all ages in their place of residence, and coordinate care with the interdisciplinary team, patient / family, and referring agency.

Licensed Vocational Nurse / LVN Essential Functions

• Under the direction of the RN, participate in planning and implementing specialized procedures and duties that assist in meeting the nursing care needs of the patient by agency policies
• Assist the RN in carrying out the plan of care
• Assist the patient in learning appropriate self-care techniques
• Complete appropriate documentation promptly
• Provide care utilizing infection control measures that protect both staff and patient (OSHA)
• Provide effective communication to patient / family, team members, and other health care professionals
• Monitor assigned cases to ensure compliance with requirements of third-party payor
• Demonstrate commitment, professional growth, and competency
• Promote Agency philosophy and administrative policies to ensure the quality of care

Education : Graduate of an accredited school of vocational nursing

Reliable transportation. Valid and current auto liability insurance

Works in patients' homes in various conditions; possible exposure to blood and bodily fluids and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather; PRN emergency call.
